Wally Schirra, first person to fly to space 3 times, signed maintenance report twice in pencil on March 29, 1961 - once under 'Exceptional Release' and once under 'Pilot's Signature'. Flight report was for F-106A training at Langley AFB in Virginia, where most Mercury astronaut flight training occurred. Second is attached discrepancy report (two pages), listing problems, who discovered them and who corrected and inspected. Both forms are fine, with edge wear and some toning
